Participants in our instruction and canoe/kayak rental programs are required to sign a Participant Agreement and a Medical Information Form . Please link to these documents and review them. These agreements may not be modified or amended. If you are mailing in your registration form, you may print these forms and include them. If you are faxing your registration or registering over the phone, copies of the agreements will be emailed to you, along with payment confirmation and event information. Please postal mail them back to us or bring them to the class. (Our insurance company requires original signatures, so do not fax or email them back to us.) In general, rental customers will receive the Participant Agreement to sign along with the Rental Agreement and credit card slip when equipment is picked up or delivered.
